Care Line Offers Continuum for Wide Range of Older Adults' Needs

Often when people think of Thompson Health, they think of the hospital and perhaps its Urgent Care Center or family practices.

Yet even among our own medical staff, I've found a need for greater awareness of the health system's Senior Living Services. This care line offers a continuum that can meet the needs of many of the older adults you may treat.

The largest component of this continuum is the M.M. Ewing Continuing Care Center (CCC), the 188-bed skilled-nursing facility adjacent to the hospital.

In addition to providing 24-hour care, the CCC has two settings designed for individuals with dementia (including advanced-stage). With an active Rehabilitation Services team, the CCC also offers post-hospital services for people who are recovering from illness, injury or surgery such as a total joint replacement.

While many of you may be familiar with the CCC, I imagine fewer members of our medical staff are aware of The Brighter Day, the CCC's off-site medical adult day program. Now in its 21st year, the program provides medical supervision and socialization during weekdays for individuals who live at home alone, in group homes or with caregivers.

With an RN on site at all times, The Brighter Day's services include memory/dementia care, wound care, phlebotomy services, urostomy/colostomy care and tracheotomy care, as well as management of conditions including COPD, diabetes and psychiatric disorders.

Those who can still live independently-with a structured system of prompts and cues-make up the resident base of Clark Meadows at Ferris Hills, Thompson's 48-apartment enriched living community in Canandaigua.

LPNs and PCAs provide assistance with medication and personal care at Clark Meadows, where residents receive meals in a restaurant-style setting, as well as housekeeping, recreational activities and transportation to and from appointments.

Clark Meadows offers a "short stay" program for those recuperating from injury, surgery or illness. This program is also used when caregivers take a vacation or business trip, or when they simply need respite. It has proven to be an effective, comfortable way to introduce older family members to enriched living.

Meanwhile, Ferris Hills at West Lake is Thompson's community for seniors who remain active and independent but may receive peace of mind from amenities including a 24-hour call system.
